is growing and we are seeking Full Time RN field case manager in Wimberley, TX area to aide in delivering high quality care to our patients and families.
Duties consist of performing Hospice admissions, supervision of LVN and Aides, participating in IDT meetings and managing a patient case load of up to 14 patients.
